WSSCA Membership Application and Endorsement

 

  • Download and print form.
    • Current Adobe PDF Reader required.
    • Place printer in Portrait Mode and set Margins to Minimum.

  • Applicant completes the first three pages of the form and forwards the form with your check for the Membership Fees to your sponsor. (Make check out to WSSCA, Inc.)

  • The Sponsor completes Page 4 of the Membership Application/Endorsement Form and forwards the completed form and applicants check to the WSSCA Corresponding Secretary at the address listed on the form.

  • If you encounter any problems please notify the webmaster so that the problem can be investigated.
